FibroScan

What Is FibroScan?

FibroScan is a non-invasive liver assessment used to evaluate liver stiffness and, in appropriate cases, estimate the degree of liver fibrosis. It can also provide an assessment of liver fat through a measurement known as the Controlled Attenuation Parameter (CAP).

Unlike a conventional liver biopsy, FibroScan does not require a tissue sample. The examination uses specialized ultrasound-based technology to assess the physical properties of the liver.

FibroScan can be particularly useful when evaluating patients with fatty liver disease, chronic liver disease, viral hepatitis, alcohol-related liver disease and other conditions that may cause progressive liver scarring.

What Does a FibroScan Measure?

A FibroScan examination can provide two important types of information:

Liver Stiffness Measurement

Liver stiffness can increase when fibrosis or scarring develops in the liver. FibroScan measures liver stiffness to help the gastroenterologist assess the likelihood and extent of liver fibrosis.

The result needs to be interpreted in the context of the patient’s medical history, symptoms, blood tests and other investigations.

Controlled Attenuation Parameter (CAP)

CAP is used to assess the amount of fat in the liver. It can support the evaluation of hepatic steatosis, commonly referred to as fatty liver.

FibroScan therefore provides useful information about two important aspects of liver health: fat accumulation and liver stiffness.

When Is FibroScan Recommended?

A gastroenterologist may recommend FibroScan when there is a need to assess liver health or monitor an established liver condition.

It may be considered in patients with:

  • Fatty liver disease
  • Suspected liver fibrosis
  • Chronic hepatitis B or hepatitis C
  • Alcohol-related liver disease
  • Abnormal liver function tests
  • Metabolic conditions associated with fatty liver
  • Suspected chronic liver disease
  • Known liver disease requiring monitoring
  • Previous investigations suggesting liver damage or fibrosis

FibroScan is generally considered alongside other clinical information rather than being used as an isolated test.

FibroScan for Fatty Liver

Fatty liver is increasingly recognized in people with metabolic risk factors such as obesity, diabetes, insulin resistance and abnormal cholesterol levels.

While detecting fat in the liver is important, determining whether significant fibrosis is also present can provide additional information about long-term liver health.

FibroScan can assess liver fat using CAP and liver stiffness using a stiffness measurement. This can help the treating gastroenterologist determine whether further evaluation or closer monitoring may be appropriate.

FibroScan for Liver Fibrosis

Liver fibrosis develops when repeated or ongoing liver injury causes scar tissue to form.

Fibrosis may occur in conditions such as:

  • Chronic viral hepatitis
  • Fatty liver disease
  • Alcohol-related liver disease
  • Certain metabolic or autoimmune liver conditions
  • Other chronic liver disorders

FibroScan provides a non-invasive way of assessing liver stiffness, which can help in the evaluation of fibrosis.

Importantly, a FibroScan value should not be interpreted independently. Liver stiffness can be influenced by several factors, so the result should be assessed together with clinical findings and other investigations.

How Is FibroScan Performed in Indore?

FibroScan is a relatively simple examination.

During the assessment:

  1. You will generally be asked to lie comfortably on your back.
  2. The examination area over the liver is exposed.
  3. A probe is placed against the skin over the right upper abdomen.
  4. The device sends controlled vibrations through the liver.
  5. The resulting measurements are collected by the FibroScan system.
  6. The readings are reviewed and interpreted in the appropriate clinical context.

There is no incision and no tissue removal during the examination.

Is FibroScan Painful?

FibroScan is generally a non-invasive and well-tolerated examination.

Patients may feel the probe against the skin and a series of mild vibrations during the measurements. There is no surgical incision or needle biopsy involved.

If you have concerns about the examination, your gastroenterologist can explain the process and what to expect before the assessment.

How Should You Prepare for FibroScan?

Preparation requirements can vary depending on the clinical situation and the facility performing the examination.

Patients are commonly advised to follow fasting instructions before the examination because food intake can affect certain liver-related measurements.

Before your appointment, follow the specific preparation instructions provided by your healthcare team.

You should also inform the doctor about your existing medical conditions, medications and previous liver investigations.

Understanding FibroScan Results

FibroScan results are usually expressed using measurements of liver stiffness and, when available, CAP.

A higher liver stiffness measurement may suggest a greater likelihood of significant liver fibrosis, but the result does not automatically establish a specific stage of liver disease.

Several factors can influence liver stiffness measurements. Therefore, the doctor may consider:

  • FibroScan findings
  • Liver function tests
  • Viral hepatitis testing where relevant
  • Ultrasound or other imaging
  • Symptoms and medical history
  • Metabolic risk factors
  • Previous liver investigations
  • Clinical examination

This combined assessment helps determine whether additional testing, monitoring or treatment is appropriate.

FibroScan and Liver Biopsy

FibroScan and liver biopsy are different investigations.

A liver biopsy involves obtaining a small tissue sample from the liver for microscopic examination. FibroScan, in contrast, provides a non-invasive assessment of liver stiffness and can also assess liver fat through CAP.

FibroScan may therefore be useful when repeated or non-invasive assessment of liver health is required. However, it does not provide the same type of microscopic information as a biopsy.

Whether a liver biopsy or another investigation is necessary depends on the individual clinical situation.

Dr. Amol Patil is a Senior Consultant Gastroenterologist & Hepatologist with more than 15 years of clinical experience and expertise in complex GI and liver disorders. His training includes DM (Gastroenterology) from CMC Vellore and a Fellowship in Advanced Endoscopy.

His practice includes evaluation and management of disorders involving the liver, digestive tract, pancreas, gallbladder and bile ducts. His hepatology expertise allows liver-related findings to be assessed as part of the patient’s overall clinical picture rather than in isolation.

State-of-the-Art Gastroenterology & Liver Care in Indore

A comprehensive liver evaluation may involve more than one investigation.

Depending on the patient’s condition, evaluation may include blood tests, abdominal imaging, FibroScan, endoscopic assessment or other investigations.

The objective is to identify the underlying cause of liver abnormalities, assess the extent of liver involvement and develop an appropriate management plan.

Personalized Patient Care

Liver conditions can vary considerably between individuals. A FibroScan result that is appropriate for one clinical situation may require a different interpretation in another.

For this reason, Dr. Amol Patil evaluates FibroScan findings alongside the patient’s symptoms, medical history, laboratory results, imaging and other relevant clinical information.

This approach helps provide personalized evaluation and evidence-based management for liver and digestive disorders.

When Should You See a Gastroenterologist?

Consider consulting a gastroenterologist if you have persistent or unexplained liver-related abnormalities, fatty liver, abnormal liver function tests or a known chronic liver condition.

A specialist evaluation can help determine whether FibroScan or another liver investigation is appropriate.
